We are pleased that you are considering Mt. Juliet Christian Academy for your child's education. The following items must be submitted before your child's application can be considered and an admissions decision made:
1. Completed Mt. Juliet Christian Academy Application with signature.
2. Registration Fee (non-refundable).
3. Copy of Birth Certificate.
4. Tennessee State School Immunization Form.
5. Copy of Social Security Card.
6. Two Letters of Recommendation.
7. Pastor Recommendation Form (Elementary and Secondary Only).
8. Copy of Most Recent Report Card (Elementary and Secondary Only).
9. Copy of Most Recent Achievement Test Scores (Elementary and Secondary Only).
10. Emergency Medical Form (with Transportation Pick-up information on back).
11. Authorization for Administration of OTC/RX Medications Form (requires a physician's signature).
12. Self-Carry Medication Permission form (do not fill out this form unless your child needs an EpiPen, inhaler, etc.).
It is the responsibility of the parent/guardian to obtain and submit the required application materials. We will be happy to request records or make copies for you. Your application cannot be processed for admission until the above have been submitted.
